RESUMO

Resorptive cells are responsible for the resorption of mineralized matrix of hard tissues. Bone-resorbing cells are called osteoclasts; however, they can resorb mineralized dental tissues or calcified cartilage and then they are called odontoclasts and chondroclasts, respectively. Resorptive cells form when mononuclear precursors derived from a monocyte-macrophage cell lineage are attracted to certain mineralized surfaces and subsequently fuse and adhere onto them for exerting their resorbing activity. These cells are responsible for degradation of calcified extracellular matrix composed of organic molecules and hydroxyapatite. The activity of these cells can be observed in both physiological and pathological processes throughout life and their activity is mainly required in bone turnover and growth, spontaneous and induced (orthodontic) tooth movement, tooth eruption, and bone fracture healing, as well as in pathological conditions such as osteoporosis, osteoarthritis, and bone metastasis. In addition, they are responsible for daily control of calcium homeostasis. Clastic cells also resorb the primary teeth for shedding before the permanent teeth erupt into the oral cavity.

OBJECTIVES: Prognosis of the avulsed teeth is mostly affected by extraoral dry period and storage medium used to store teeth before reimplantation. However, ability of storage media can affect cell viability and success of treatment. Various storage media were tried with some success. The present study was undertaken to comparatively evaluate the efficacy of hank's balanced salt solution (HBSS), propolis, Aloe vera, and pomegranate juice (PJ) in preserving the vitality of periodontal ligament (PDL) cells of avulsed teeth. MATERIALS AND METHODS: Fifty orthodontically extracted sound teeth with healthy PDL were selected for the present study. Selected teeth were randomly divided into study groups (10 in each) and 5 each as positive and negative control groups. All the teeth were immersed immediately after extraction into respective storage media. Data were statistically analyzed using IBM SPSS software for Windows, Version 19.0., IBM Corp., Armonk, NY, USA. Analysis of variance and multiple range were done using Tukey's honestly significant difference with level of significance at 5% (P > 0.05). RESULTS: Propolis (285,000 viable cells with standard deviation 4.11028 and standard error of 1.38097) showed more viable PDL cells followed by HBSS, A. vera, and PJ. CONCLUSION: Propolis, A. vera, and PJ can be used as an alternative tooth storage media.

BACKGROUND: Posts are used to enhance crown buildup in pulpless teeth with destructed crown portion. Different types of post are used in endodontically treated teeth. The aim of the present in vitro study was to evaluate fracture resistance of custom made, metal, glass fiber reinforced and carbon reinforced posts in endodontically treated teeth. MATERIALS AND METHODS: An in vitro study was carried out on extracted 40 human maxillary central incisor teeth, which was divided into four groups with 10 samples in each group with custom made, metal post, glass fiber reinforced, and carbon reinforced posts. The samples were decoronated at cemento-enamel junction and endodontically treated. Post space was prepared and selected posts were cemented. The composite cores were prepared at the height of 5 mm and samples mounted on acrylic blocks. Later fracture resistance to the compressive force of samples was measured using Universal Testing Machine. RESULTS: The maximum resistance to the compressive force was observed in carbon reinforced and glass fiber reinforced posts compared others which is statistically significant (P > 0.001) and least was seen in custom fabricated post. CONCLUSION: It is concluded that carbon reinforced fiber post and glass fiber posts showed good fracture resistance compared to custom made and metal posts.

BACKGROUND: The aim of this study was to quantitatively analyze the amount of dye leakage with AH26, Sealapex and Tubliseal sealers in endodonticaly treated teeth. MATERIALS AND METHODS: A total of 36 extracted mandibular molar specimens were divided into three groups; Group I: Sealapex, Group II: Tubliseal, Group II: AH26 with 12 samples in each group. Standard access cavity and biomechanical preparation was done with step back flare technique. Obturations were done using respective sealers in the three different groups. Then, samples were subjected to spectro photometric analysis using a filter of 670 nm. Spectrophotometric analysis was performed to quantitatively analyze the amount of dye leakage with all three sealers. RESULTS: Tubliseal exhibited the least microleakage. In comparison to Sealapex, Tubliseal showed a significant difference. Compared to Sealapex, AH26 showed no significant difference. Comparisons made between Tubliseal and AH26 showed no significant difference. CONCLUSION: In the present study, Tubliseal sealer showed least microleage compared with Sealapex and AH26 sealer.

Hemisection is sectioning of multi-rooted teeth with its crown portion, with the loss of periodontal attachment and is performed to retain the original tooth structure and attain the fixed prosthodontic prosthesis. We describe hemisection of a mandibular molar tooth followed by adequate restoration in a young patient.

OBJECTIVES: Acquired Immunodeficiency Syndrome (AIDS) is a highly lethal, progressively epidemic viral infection characterized by profound impairment of the immune system. Oral manifestations are common in Human Immunodeficiency Virus (HIV) infected AIDS patients, and are usually the first indicator of symptom and disease progression. The main objective of the current study was to compare the prevalence of oral manifestations in HIV patients on Highly Active Antiretroviral Therapy (HAART) with those, not on HAART therapies. MATERIALS AND METHODS: A cross sectional study was conducted among 100 patients diagnosed as human immune virus sero-positive. These patients were divided equally into two groups (50 each); Group I patients on HAART and Group II patients who were not on HAART. Information regarding age, sex and cluster of differentiation 4 cell count was obtained from the medical records. Oral examination was done, and findings were recorded by using internationally accepted presumptive clinical criteria. Statistical analysis was performed using Chi-square statistical test. RESULTS: The presence of oral manifestations was significantly decreased in subjects on HAART (32%) compared to those who are not on HAART (56%). The most common oral lesions detected in patients on HAART were increased oral hyper-pigmentation (14%), recurrent aphthous stomatitis (8%), non-specific ulcerations (4%), pseudo-membranous candidiasis (2%), periodontitis (2%) and xerostomia (2%), whereas in non HAART oral hyperpigmentation (10%), pseudo-membranous candidiasis (8%), angular cheilitis (4%), and erythematous candidiasis (4%) and Periodontitis (14%) were more prevalent. CONCLUSION: The number and severity of oral manifestation decreased, and even there was a change in the type of oral manifestations on HAART, which may be because of the improvement in immunity gained by the therapy.

BACKGROUND: Periodontitis is an inflammatory disease resulting in the destruction of periodontal tissues. Various treatment modalities have been tried in the form of mechanical therapy and surgical therapy. Antimicrobial agents have been used as a monotherapy and as an adjunct with mechanical debridement. Various plant extracts have been used as antibacterial agents. Pineapple extract (bromelian) is one such agent. Hence this study was conducted to assess the antibacterial efficacy of bromelain on both aerobic and anaerobic periodontal microorganisms. The aim was to assess the antibacterial efficacy of bromelain on both aerobic and anaerobic periodontal microorganisms. MATERIALS AND METHODS: Minimum inhibitory concentration (MIC) of bromelain was tested on isolated strains of Streptococcus mutans, Enterococcus fecalis Aggregatibacter actinomycetemcomitans (Aa), and Porphyromonas gingivalis (Pg) using serial dilution broth method. RESULTS: S. mutans showed sensitivity at the lowest concentration of 2 mg/ml as compared to E fecalis (31.25 mg/ml) while Pgingivalis showed sensitivity at the lowest concentration of 4.15 mg/ml as compared to Aa (16.6 mg/ml). CONCLUSION: Bromelain exerts an antibacterial effect against potent periodontal pathogens; hence, it may be used as an antibacterial agent. However, further trial has to be conducted to validate this result.

Juvenile ossifying fibroma is an uncommon fibro osseous neoplasm at a younger age. It is a well-defined clinical and histological lesion. This lesion is locally invasive and spreads quickly. Early detection and complete surgical excision of this lesion is essential since it has high recurrence rate.

OBJECTIVE: The aesthetics of the patient can be improved by surgical reconstruction of interdental papilla by using an advanced papillary flap interposed with subepithelial connective tissue graft. MATERIALS AND METHODS: A total of fifteen sites from ten patients having black triangles/papilla recession in the maxillary anterior region were selected and subjected to presurgical evaluation. The sites were treated with interposed subepithelial connective tissue graft placed under a coronally advance flap. The integrity of the papilla was maintained by moving the whole of gingivopapillary unit coronally. The various parameters were analysed at different intervals. RESULTS: There was a mean decrease in the papilla presence index score and distance from contact point to gingival margin, but it was statistically not significant. Also, there is increase in the width of the keratinized gingiva which was statistically highly significant. CONCLUSION: Advanced papillary flap with interposed sub-epithelial connective tissue graft can offer predictable results for the reconstruction of interdental papilla. If papilla loss occurs solely due to soft-tissue damage, reconstructive techniques can completely restore it; but if due to periodontal disease involving bone loss, reconstruction is generally incomplete and multiple surgical procedures may be required.

PURPOSE: To evaluate the antimicrobial activity of different compositions of Gutta-percha points and calcium hydroxide (Ca(OH)2) pastes, used in endodontic therapy. MATERIALS AND METHODS: The evaluated material consisted of Gutta-percha points containing Ca(OH)2, Gutta-percha points containing chlorhexidine (Chx), conventional Gutta-percha points and Ca(OH)2 pastes. Antimicrobial properties of Chx and CaOH paste are compared with CaOH points. Antimicrobial tests included three species of microorganisms: Escherichia coli (ATCC 25923), Staphylococcus aureus (ATCC 25922) Pseudomonas aeruginosa (ATCC BAA-427), the agar disc diffusion method was employed. The plates were kept at room temperature for 2 h for prediffusion and then incubated at 37°C for 24 h. Zones of inhibition were measured. RESULTS AND CONCLUSION: All microbial species used in the study were inhibited by the Gutta-percha points containing Chx and by the Ca(OH)2 pastes, no antimicrobial activity was observed for the other groups (conventional Gutta-percha and Ca(OH)2 group).

BACKGROUND: The present study was done to evaluate the dimensional stability and surface quality of Type IV gypsum casts retrieved from disinfected elastomeric impression materials. MATERIALS AND METHODS: In an in vitro study contaminated impression material with known bacterial species was disinfected with disinfectants followed by culturing the swab sample to assess reduction in level of bacterial colony. Changes in surface detail reproduction of impression were assessed fallowing disinfection. RESULTS: All the three disinfectants used in the study produced a 100% reduction in colony forming units of the test organisms. CONCLUSION: All the three disinfectants produced complete disinfection, and didn't cause any deterioration in surface detail reproduction. BACKGROUND: Traumatic injuries caused to anterior teeth are most common. Emergency management of fractured fragments is necessary, for preserving their vitalities and for retaining aesthetics in an economical way. Various methods are available for restoring fractured, uncomplicated teeth, such as reattachment of fractured fragments, composite restoration. But only limited data is available on evaluation of the strength of reattached fractured fragments. Hence, the present study was designed. OBJECTIVES: The objective of this study was to evaluate the fracture strength recovery of re-attached anterior fractured tooth fragment by using different re-attachment techniques. METHODOLOGY: Forty human upper central incisions were used in this study. The roots of the teeth were confined in a special device (holder) and adapted in a Universal Testing Machine. Load was applied to each tooth in bucco-lingual direction, by using a small stainless steel ball. The force which was required to fracture the tooth was recorded. Both the fragment and remaining fractured tooth was restored by using four reattachment techniques - simple reattachment, external chamfer, over contour and internal dentinal groove. Specimens were loaded in same pre-determined area which was used in procedure to obtain fragments. The force required to detach each fragment was recorded and it was correlated with the fracture strength of an intact tooth and that which was obtained after doing restorative procedures for all groups i.e. fracture strength recovery. RESULTS: Technique I (simple reattachment) and Technique 2 (external chamfer) showed fracture strength recoveries of 44.3% and 60.6% respectively. However, these values were lower than those which were obtained by usingTechnique 3 (Over contour) -86.8% and Technique 4 (internal dentinal groove) -89.5%. CONCLUSION: Over contour and internal dentinal groove reattachment is a preferred technique as compared to the other reattachment techniques which were tested.

BACKGROUND: Internet is a useful tool to update the knowledge. The aim of the present study was to assess the current level of knowledge on the computer and internet among under graduate dental students. MATERIALS AND METHODS: The study consists of self-administered close ended questionnaire survey. Questionnaires were distributed to undergraduate dental students. The study was conducted during July to September 2012. RESULTS: In the selected samples, response rate was 100%. Most (94.4%) of the students had computer knowledge and 77.4% had their own computer and access at home. Nearly 40.8% of students use computer for general purpose, 28.5% for entertainment and 22.8% used for research purpose. Most of the students had internet knowledge (92.9%) and they used it independently (79.1%). Nearly 42.1% used internet occasionally whereas, 34.4% used regularly, 21.7% rarely and 1.8% don't use respectively. Internet was preferred for getting information (48.8%) due to easy accessibility and recent updates. For dental purpose students used internet 2-3 times/week (45.3%). Most (95.3%) of the students responded to have computer based learning program in the curriculum. CONCLUSION: Computer knowledge was observed to be good among dental students.

AIM: This study was aimed to evaluate the antibacterial effect of calcium hydroxide (CH) in different vehicles in an in vitro model. MATERIALS AND METHODS: Calcium hydroxide paste prepared with two conventionally used vehicles namely, camphorated monochlophenol, distilled water and propylene glycol. The antibacterial activity of these paste were tested against five micro-organisms that can commonly occur in the infected root canals. RESULTS: The results of this study indicate that a paste of CH made with propylene glycol exerts significant antibacterial action. Hence, it can be recommended for use as an intracanal medicament in preference to a paste prepared with a tissue toxic phenolic compound like camphorated monochlorophenol.

BACKGROUND: The most common cause of failure of endodontic therapy is inadequate apical and coronal seal. Proper coronal seal reduces the risk of endodontic failure. Hence, the present study was done to test the role of self-etching primers in reducing microleakage through coronal seal. MATERIALS AND METHODS: Following root canal preparation and obturation, 46 specimen teeth were subjected to one of the test methods as follows: Group I - deproteinization with 3% sodium hypochlorite and etching with 37% phosphoric acid; Group II - deproteinization with 3% sodium hypochlorite and chelation with 15% ethylenediaminetetraacetic acid (EDTA) (Glyde) without etching. Group I and Group II were further divided into two subgroups with 10 specimens in each: In subgroup A, Clearfil Liner Bond 2V was used and in subgroup B, Excite was used. Group III (obturated without access restorative material) had six specimens. RESULTS: Spectrophotometric analysis was done to quantitatively analyze the amount of dye leakage. Microleakage values obtained in Group I and Group II were comparable. In Group I, marginally better values were obtained with the Clearfil Liner Bond 2V in comparison with Excite. In Group II, microleakage values obtained with Clearfil Liner Bond 2V and Excite were similar and statistically not significant. In Group III (control) where no access restoratives were placed, maximum leakage was observed. CONCLUSIONS: Maximum leakage values were observed in Group III, when obturated without access restorative and when exposed to artificial saliva. Clearfil Liner Bond 2V as a self-etching primer showed better values in preventing microleakage. Deproteinization may be important to reduce microleakage when using the fifth-generation bonding system (Excite) and sixth-generation bonding system (Clearfil Liner Bond 2V).

OBJECTIVE: The aim of this study was to evaluate the influence of using the dental operating microscope (DOM) and ultrasonics for the detection of second mesiobuccal (MB2) canal orifice in maxillary first molars. MATERIALS AND METHODS: Sixty subjects seeking root canal therapy for maxillary first molar were assessed for the presence of MB2 canal using endodontic explorer without magnification. Teeth in which the MB2 canal orifice could not be located were examined under magnification using DOM. If the MB2 canal orifice could not be found even after using DOM, ultrasonic tips were used to prepare 3-mm-long trough from the mesiobuccal canal orifice toward the palatal canal and examined under DOM for location of the canal. RESULTS: With naked eye, the MB2 canal was located in 12 teeth; with the use of the DOM, the MB2 canal was located in 21 additional teeth; and with the combined use of ultrasonic tip and DOM, the MB2 canal was located in 9 more teeth. Statistical comparisons between the tested techniques were done by analyzing the Receiver Operating Characteristic (ROC) curves; a statistically significant difference was found (P < 0.001). CONCLUSION: The results of this study indicate that the DOM and ultrasonics provide increased opportunity for the dentist to detect canal orifices.

Several procedures are advised to manage fractured anterior tooth structure using acrylic resin, composite restoration, ceramic or metal crown with ceramic facing. Biologic restoration is a procedure to restore fractured tooth structure with natural tooth material. In this in vitro case we have made an attempt for aesthetic rehabilitation of maxillary central incisor with similar biologic crown taken form extracted maxillary central incisor. It was observed that biologic restoration is an aesthetic, economical, fast and functional procedure which can be used as an alternative method to restore fractured primary or permanent anteriors.

BACKGROUND: Presence of plaque may be the culprit for dental caries, gingivitis, periodontal problems, and halitosis. Many mechanical aids are practiced worldwide to remove or control plaque, including tooth brushes, dental floss, mouth rinses, and dentifrices. The objective of this clinical study was to investigate the effectiveness of herbal toothpaste (Dabur Red) in controlling plaque and gingivitis, as compared to conventional (non-herbal) dentifrice (Pepsodent). MATERIALS AND METHODS: In this study, 30 subjects aged 35-43 years with established gingivitis and at least 20 natural teeth, and having a probing depth <3 mm were investigated. After the washout period, plaque and gingival index (PI and GI, respectively) scores were assessed at days 0 and 30. Differences between groups were compared with Mann-Whitney U test and the mean scores of PI and GI by Wilcoxon test. Statistical difference between the weights of dentifrices tubes on days 0 and 30 was evaluated by Student's t-test. RESULTS: At the end of 30 days of the study, there was statistically significant difference between both the groups for plaque and gingival scores. CONCLUSION: After 30 days of trial, both test and control groups showed effective reduction of plaque and gingivitis, which was statistically significant. No adverse reactions to dentifrices products were observed during the trial. It was concluded that herbal dentifrice was as effective as non-herbal dentifrices in the control of plaque and gingivitis.

RESUMO

Oral squamous papillomas are benign proliferating lesions induced by human papilloma virus. These lesions are painless and slowly growing masses. As an oral lesion, it raises concern because of its clinical appearance. These lesions commonly occur between age 30 and 50 years, and sometimes can occur before the age of 10 years. Oral squamous papilloma accounts for 8% of all oral tumors in children. Common site predilection for the lesion is the tongue and soft palate, and may occur on any other surface of the oral cavity such as the uvula and vermilion of the lip. Here, we are presenting a case of squamous papilloma on the palate.
